Hip-replacement-Wo-Insurance

Paying for surgery from your own pocket, known as self-paying, can make a procedure expensive for some people. If the insurance does not cover the surgery or when higher insurance premiums are unaffordable, it becomes imperative to look for the most cost-efficient hospitals and doctors.

A total hip replacement surgery can significantly improve the quality of life for a patient, relieving pain and increasing the range of movement. When the pain becomes too much and the hip starts affecting the patient’s daily life, and non-invasive treatment options such as physical therapy and steroid injections, hip replacement surgery are the most effective options.

However, the expense of the surgery without insurance coverage may cause many patients to delay treatment. This could lead to further deterioration of the hip condition and associated symptoms. In such situations, hip replacement in India can be a cost-effective and safe option for the treatment of hip arthritis or pain.

Not only will a patient save money on the overall treatment for hip replacement surgery without insurance, but they will also have the benefits of the best services, the latest surgical technology, and no waiting period for their surgery.

Low-cost hip replacement without insurance

Hip replacement is a major surgical procedure that can be considerably expensive without an insurance pay-out. But this doesn’t mean the patient should avoid surgery. There are many less expensive and cost-saving ways to have the surgery within the budget.

India has a number of government and private hospitals that are renowned multi-specialty tertiary medical centers. Daycare hip replacement cost starts from $3000 at the top hospitals in Delhi. Furthermore, the cost of a hospital stay is also reduced. It is available for select patients who are fit for the surgery. The standard hip replacement cost in India ranges from $3000 to 8000. The package price may vary based on various factors, including:

  • Choice of a hospital: The location, accreditation and overall standard of the hospital influence the package cost. The internationally accredited hospital in Delhi, Mumbai, Bangalore, Chennai and other major cities are world-class centers that provide impeccable services to the patient. Therefore, the cost might be more in comparison to a hospital in minor cities and without accreditations.
  • Type of hip replacement: The price for a single hip replacement is lower than the bilateral hip replacement surgery. The doctor will evaluate the patient’s condition and recommend the best type of hip replacement surgery based on it.
  • Quality of implants: A variety of hip replacement prosthetics are available based on the type of material and manufacturers. Their prices vary and the choice of implant is determined based on the patient’s age, overall health, medical condition and doctor’s recommendation.
  • Type of technology used: Advanced surgical systems such as NAV3 computer navigation for hip replacement and robot-assisted minimally invasive surgery may cost more than the standard open approach. However, these methods offer the advantage of faster recovery, less post-operative pain and complications, shorter hospital stay and minimal scarring.
  • Surgeon’s fee: This is also an important part of the complete package cost. The more experienced and skilled surgeons may have more charges, but their success record is also excellent.
  • Length of hospital stay: This may depend on the type of surgery and technique used, as well as the choice of room category.

An overview of Hip replacement surgery

Hip replacement surgery is an operation in which the damaged hip joint of the patient is replaced with an artificial prosthetic implant. A hip replacement can be performed as a total hip replacement or a partial replacement, depending on the damage in the joint.

The aim of joint replacement surgery is to relieve pain from arthritis or hip fractures. A total hip replacement is also known as total hip arthroplasty and involves the replacement of the complete hip joint with prosthetic parts to enable smooth movement of the ball and socket joint without pain. Hip replacement is one of the most commonly performed orthopedic surgeries and has a very good success rate, typically more than 99%.

Why need a hip replacement?

Chronic hip pain can be caused by a variety of factors and reasons. However, the most common one is arthritis in the hip joint, which leads to the need for hip replacement in certain conditions. There are different types of arthritis for which hip replacement surgery can be considered for patients who meet the standard criteria.

Osteoarthritis: This type of arthritis is related to age-related wear and tear of the joint. It is typically observed in people 50 years of age and older. Also, the condition is often found in individuals with a family history of arthritis. In this, the cartilage that cushions the bones of the hip wears away, causing the bones to rub against each other. This leads to hip pain and stiffness.

  • Rheumatoid arthritis: It is an autoimmune disease that results when the synovial membrane becomes inflamed and thickened. Chronic inflammation can cause damage to the cartilage, leading to pain and stiffness in the joint. This is the most common type of condition from a group of disorders termed inflammatory arthritis.
  • Post-traumatic arthritis: This can be caused by a major hip injury or fracture. The injury may lead to damage in the cartilage, leading to hip pain and stiffness over time.
  • Osteonecrosis: A hip injury due to dislocation or fracture may lead to restriction of the blood supply to the femoral head. This condition is known as osteonecrosis. It is also sometimes referred to as avascular necrosis. The limited amount of blood may cause the surface of the hip bone to collapse, resulting in arthritis. Certain diseases can also lead to osteonecrosis damage.
  • Some childhood hip diseases: Hip problems that begin in infants and children may still cause arthritis in the later stage of life. Even though the problem is usually successfully treated during childhood, it can flare up in adulthood or older age. This is mainly because the hip does not grow normally, and the joint surfaces get affected.

In addition, the indications for hip replacement surgery generally include, but are not limited to, the following:

  • Chronic pain in the hip joint
  • Almost no relief from pain medications or therapies
  • Trouble walking or performing normal activities such as climbing stairs and squatting in daily life
  • Pain when rising from a sitting position
  • Not being able to do certain activities because of severe and persistent pain

An orthopedic surgeon evaluates the patient to see damage to cartilage surrounding the joint area. This includes physical examination and some imaging tests. Based on the damage and symptoms, the doctor recommends the appropriate type of treatment.

Types of hip replacement

  • Total hip replacement: This is the most commonly performed type of hip replacement.
  • In this surgery, the surgeon removes the worn-out or damaged sections of the hip and replaces them with artificial implants. The socket part of the hip joint is replaced with a durable plastic cup, with or without a titanium metal shell. The femoral head in the joint is removed and replaced with a ball prosthetic that can be made from ceramic or a metal alloy. Then, the ball implant is attached to a metal stem which is inserted on the top part of the femur. This forms a complete replacement of the joint and enables proper movement of the hip.
  • Partial hip replacement: This is known as hemiarthroplasty. It involves the replacement of only one part of the hip joint. The femoral head is removed and replaced, instead of both sides which is typically done in a total hip replacement surgery. This procedure is often indicated for older patients who have a fractured hip.
  • Hip resurfacing surgery: This type of hip surgery is most commonly done in patients who are younger and more active. Hip resurfacing is aimed at relining rather than replacing the worn-out part of the hip joint. This technique involves a bigger hip ball than generally used in hip replacement surgery. It may help reduce the risk of dislocation. Hip resurfacing is more suited for younger patients who are not morbidly obese and qualify the criteria for a hip replacement. In addition, people who have the non-inflammatory degenerative joint disease but no infection or allergy to the metal parts in an implant.

How is hip replacement surgery performed?

Hip replacement is performed while the patient is unconscious under the effects of general anesthesia or sedated with spinal anesthesia. The choice of anesthesia will depend on the doctor and patient’s preference. The anesthesiologist will discuss this prior to the surgery.

There are two major surgical approaches for performing a total hip replacement – the posterior approach, which is more common and the anterior approach.

Generally, hip replacement surgery involves the following steps:

  • The skin over the hip is cleaned with an antiseptic solution and the surgeon makes an incision in the surgical area. The length of the incision will depend on the type of technique being used for the surgery. Minimally invasive surgery involves smaller incisions than the standard procedure and special instruments are used to perform the operation.
  • The damaged parts of the hip joint are removed and replace with the metal or plastic implant. The hip prosthesis involves a stem that is inserted into the thighbone or femur and the head joint or ball that fits into the stem. A cup is inserted into the socket of the hip joint with the ball made of metal or ceramic. The cup has a liner that can be made of plastic or ceramic.
  • The two most common types of artificial hip prostheses are used are cemented hip implant and uncemented hip implant. A cemented prosthesis is attached to the bone using surgical cement while an uncemented hip prosthesis is attached to the bone with a porous surface. The bone grows into the surface and attaches to the prosthesis. Some surgeons use a combination of both of these types to replace a hip.
  • Once all the implant parts have been placed, the incision is closed with stitches or surgical staples and a drain is placed in the incision site to remove the fluid.
  • A dressing or bandage is put to cover the incision site while the patient recovers.

What is hip replacement surgery recovery like?

The rehabilitation process starts within the next day after the surgery. Usually, the hip replacement patients are guided to sit and get out of the bed, and then gradually progress to walking with a cane, walker, or crutches after a day or two. The distance and frequency of walking are slowly increased.

A physical therapist is involved in this process and guides the patient through every step. The patient is given medications such as pain killers and antibiotics to help the pain and prevent infection. The medical team monitors the vital signs while the effects of anesthesia subside. Afterward, the patient is shifted to an inpatient recovery room for further recovery and rehabilitation.

Depending on the type of surgery and technique used, the hospital stay will range from 1 day to 3 days. The surgeon will provide guidelines about hip replacement care at home and precautions to be followed. This may involve avoiding strenuous activities, squatting, twisting and other activities for a few days.

The physical therapy outside the hospital continues for 6 to 8 weeks. Most patients are able to get back to their everyday activities and even playing most sports once their recovery is done.
