How Can Surgery Help With Erectile Dysfunction?

Erectile-dysfunction-surgery

Erectile Dysfunction can cause immense problems in a man’s life. It affects their mind, self-esteem, relationships, and overall emotional wellbeing. A good percentage of men don’t even seek treatment for ED due to an inherent embarrassment and shame around the condition. But, if erectile dysfunction is left untreated, it can cause severe comorbidities and psychological trauma.

Many factors combine and contribute to sexual arousal in men. These include the brain, hormones, blood vessels, nerves, muscles, as well as our emotions. When any of these factors is not in proper sync with others, a man can suffer from erectile dysfunction. In simple terms, ED can be thought of as a condition caused by underlying physical or psychological distress.

Some men may also face an inability to keep a firm erection due to anxiety, consciousness, or stress during sex. This can be quite normal and doesn’t require any medical attention. However, if this becomes persistent, an individual should look out for treatment options that suit their needs and conditions best.

Treatment options available for Erectile Dysfunction

  • Lifestyle Changes

An individual’s lifestyle and habits can have a huge impact on their sexual life. If the cause of erectile dysfunction is stress or dietary, there’s no need for a patient to look forward to any particular treatment. Adopting simple steps can help a patient to manage this condition. These changes often include eating a healthy diet, staying away from smoking or drinking, addressing relationship issues, and maintaining a work-life balance.

  • Medication

Erectile dysfunction can also occur as a result of underlying medical conditions like heart problems, diabetes, or even cancer. Undergoing a treatment or following proper medication for such conditions can also cure erectile dysfunction.

  • Vacuum erection devices

Many doctors also suggest patients to use a vacuum erection device along the course of their medication. A vacuum erection device is a cylindrical device placed around the penis, which helps to pump blood until the patient achieves an erection. It also consists of a constrictive band that holds the erection during intercourse.

  • Microsurgical Penile Revascularization

This is a surgical process suitable for patients under the age of 40 who have experienced severe trauma due to a pelvic injury. Microsurgical penile vascularization restores the blood flow to the penis. This is done by making a connection between an artery in the lower abdomen and an artery towards the top of the penis.

Some men feel that medications and other devices create a lack of spontaneity in their sexual lives. In such cases, the ideal option is to go for penile implant surgery. However, this may not be suitable for every man suffering from erectile dysfunction.

Who is an ideal candidate for penile implant surgery?

The penile implant surgery is usually reserved for patients who had undergone other ED treatments without success. However, only a few men are eligible for the procedure, including those who:

  • Have a continuous history of ED that disrupts their sex life
  • Have completed the medication course with unsatisfactory results
  • Have tried a penis pump
  • Do not lack a desire for sex
  • Do not have any Urinary tract infection
  • Do not have any skin related condition around the scrotum or the penis

What is the procedure for penile implant surgery?

Before the surgery

  • The first step of the surgery usually begins with a comprehensive physical examination conducted by the doctor.
  • After evaluating the patient’s medical history and their expectations, the doctor decides on the type of implant a patient would need.
  • There are two categories of implants that a patient can choose from, including:
  1. Malleable Implants – These implants are mechanical devices that provide permanent firmness to the penis, thus will be detectable in clothing. They are also known as semi-rigid implants.
  2. Inflatable implants – Inflatable implants can be inflated and deflated before and after sexual intercourse. They can be further categorized into three-piece and two-piece implants.

During the surgery

  • The penile implant surgery is conducted on an outpatient basis and generally lasts between 30 and 60 minutes, depending on the implant and the patient’s condition.
  • The pubic area is cleaned and shaved completely, and the patient is administered with spinal or general anesthesia.
  • A catheter and an IV line are placed to collect the urine and transport antibiotics & medications, respectively.
  • The surgeon makes an incision either at the base or just below the head of the penis. The corpus spongiosum tissue is stretched, and the implants are placed inside according to their shape, size, and type.

After the surgery

The doctor will describe a set of instructions that have to be followed by the patient for a speedy recovery. These often include:

  • Taking antibiotics to avoid the chances of infection
  • Taking medication for pain relief
  • Avoiding any form of sexual activity, including sexual intercourse and masturbation, for six weeks after the surgery
  • Avoiding strenuous exercise or physically demanding activities
  • Eating a balanced & healthy diet
  • Practicing good hygiene

How do penile implants work?

1. Three-piece inflatable pump

As the name suggests, the three-piece inflatable pump has three different components: 2 cylinders, a fluid-filled container, and an inflatable pump. During the surgery, the surgeon places these cylindrical structures inside the spongy tissue of the penis. The inflatable pump is placed in the scrotum, whereas the fluid-filled container is placed inside the lower abdomen. All these components are connected by a tube. When the pump inside the scrotum is squeezed, it causes the fluid from the container to travel towards the cylinders, causing an erection. Similarly, on squeezing the release valve, the fluid moves back to the original container, and the penis becomes flaccid.

2. Two-piece implant

This implant works similarly to a three-piece implant. However, the erection caused by the implant is less rigid. Instead of placing it in a separate container, the fluid is stored in the squeezing pump.

3. Malleable implants

This method of penile enlargement surgery involves placing two semi-rigid or flexible rods inside the tissue responsible for maintaining the erection. These implants do not change in shape or size but can be bent downwards or straightened up. Once implanted, the patient doesn’t have to worry about deflating or inflating them for sexual intercourse.

How effective is the surgery for treating Erectile Dysfunction?

  • Statistically speaking, at least 90 to 95 % of men who undergo an inflatable penile implant procedure get successful results that equate to an erection long enough to have satisfactory sexual intercourse.
  • However, it should be noted that a penile implant can only mimic an erection. These do not affect the ability of a man to feel any sensation or achieve an orgasm.
  • Plus, like any medical procedure, penile implant surgery comes with its own set of risks and complications. A patient can experience bleeding, scar tissue formation, and infection post-surgery. Mechanical issues, as well as wear & tear of the different implant parts, can also develop a need for repair surgery.
  • To conclude, penile implants are a viable option when all other treatments prove to be unsuccessful. With the right guidance and the correct choice of implant, a patient can gain back their old confident self!
