The patient is typically discharged on the same day of the procedure and experiences minimal discomfort following discharge, which is often managed with the help of medications.

Moreover, the success rate of stapedectomy in India is one of the highest in the world. More than 95 percent of the patients are successfully treated and are able to experience a significant improvement in hearing within the first week of the surgery.

Stapedectomy Surgery: An Overview

Sometimes, the bone in the ear grows abnormally around the stapes. This makes the position of the stapes fixed and results in a loss of vibration. As a result, a majority of sound vibrations do not reach properly to the inner ear. It causes hearing loss and this condition is known as otosclerosis.

Stapedectomy is performed to remove the stapes that has lost its mobility and to replace it with a prosthetic stapes bone. The artificial bone allows the sound vibrations to properly reach the inner ear and thus, restores the normal hearing ability of the patient.

Indications for Stapedectomy

Stapedectomy in India is allowed in the following cases:

  • Conductive loss in hearing due to immobilization of the stapes
  • Audiogram of the patient with conductive hearing loss shows the presence of Carhart’s notch
  • A gap of at least 30 dB in the air-bone
  • Good cochlear reserve as shown by good speech discrimination scores

Contraindications for Stapedectomy

Stapedectomy in India is not performed in the following cases:

  • If it is the only hearing ear of the patient
  • If the general condition of the patient is poor
  • Poor cochlear reserve as assessed by the presence of poor speech discrimination

What happens during stapedectomy?

Thorough knowledge of the ear anatomy is required for safe and effective stapedectomy as it is carried out through the ear canal. The surgical procedure is performed using general or local anesthesia. General anesthesia is usually preferred over local anesthesia.

The posterior canal incision is made near the ear drum using a weaver blade or a sickle knife. The tympanomeatal flap is carefully elevated without tearing the flap towards the tympanic membrane. An operating microscope is then used to see the inner ear structures in detail.

The fibrous annulus is raised over the bony annulus to reach the middle ear mucosa. The bones which are responsible for hearing are evaluated by the ear surgeon to diagnose otosclerosis. The extra deposition of calcium over the bone is usually visible and the stapes bone is tested for vibrating movement.

The stapes bone is then separated from the incus and the malleus. Both the incus and the malleus bones move on pressing after the removal of the stapes bone. Then the tendon and arch of the stapes bone are vaporized with the help of a micro instrument or laser. The stapes bone is then removed from the middle ear.

The aperture between the middle ear and the inner ear is opened. It also serves as a platform for the stapes bone. The ear surgeon then directs the laser at the aperture and a minuscule opening of approximately 0.6 mm is made.
After the opening is made, prosthetic stapes bone is inserted gently and assembled onto the incus bone. The prosthetic stapes bone is clipped with the incus bone and the vibration is checked.

Tissue from the posterior part of the ear lobe is taken to seal the aperture and space around the prosthetic stapes bone. Finally, the eardrum is positioned back to its original location and fixed with the help of small gelatin sponge.
The whole surgical procedure usually takes around 45 to 90 minutes. Stapedectomy is performed as an outpatient procedure. Some patients may, however, require an overnight stay at the hospital.

Recovery and After Care

It takes a few days for the patient to recover from the surgery. The patient should make a note of the following things after stapedectomy to recover early and avoid any complications:

  • The ear should be kept dry for at least two weeks after the surgery.
  • Not strenuous physical activity should be performed after the surgery.
  • Straining for bowel movements should be avoided.
  • The patient should also avoid blowing the nose.
  • Analgesics, antibiotics, and ear drops should be taken as advised by the ENT surgeon.

Stapedectomy: Risks and Complications

A stapedectomy performed for congenitally fixed footplate may result in hearing the loss in a greater number of patients as compared to when it is performed for otosclerosis. A few additional anomalies may be present in the case of congenital disorder, which may elevate the risk of hearing damage after the surgery. The following are some of the most common complications associated with stapedectomy:

  • Vertigo and vomiting
  • Facial palsy
  • Floating foot plate
  • Labyrinthitis
  • Tympanic membrane tear
  • Perilymph gush or fistula
  • Tinnitus

Prognosis

The success rate of stapedectomy in India is more than 90 percent. The hearing ability is improved remarkably after the surgery. Less than one percent of all the cases report no significant changes in the hearing pattern. Successful stapedectomy surgery in India improves the hearing ability of the patient by as much as 20 dB.
Some patients may report dizziness for the first two days of the surgical procedure due to an imbalance in the fluid inside the ear canal. But dizziness usually subsides within two days of the surgery. The patient may return to normal work routine after one week of the surgery. The patient is able to fly after three days of the surgery in a pressurized airplane without much inconvenience.
