What is the artificial disc made up of?

The artificial disc used for lumbar disc replacement surgery is made up of three parts. It includes two metallic plates that are meant to be placed at both ends and one central plastic core which is sandwiched between the metallic plates.

The metal endplates are usually made up of the same material, cobalt-chromium alloy, which is used to create implants for hip and knee replacement. The outer surface of the metal plates is porous, which allows firm bonding of the bone with the metal. The inner surface is made smooth to allow the swift movement with the central core. The inner surfaces are well-polished concave-shaped dishes.

The plastic used to make the central core for the total disc replacement is usually polyethylene and it is widely used in other joint replacement surgeries as well. The upper and lower surfaces of the central core are dome-shaped as they are meant to fit properly inside the concave endplate dishes.

For nucleus disc replacement, the most common material used is a hydrogel. The hydrogel is capable of expanding by absorbing water. The hydrogel device is placed in the nuclear cavity after removing the nucleus. It later expands to fill the cavity properly. The hydrogel is compressible and therefore, it allows motion much like a normal disc nucleus.

What happens during lumbar disc replacement?

Before the actual lumbar disc replacement procedure, the patient is given general anesthesia. During the surgery, the artificial disc is installed after removing the diseased disc from its place. The surgeon makes a deep incision of around 7 to 10 centimeters in the abdominal area to separate the anterior abdominal muscles. The abdominal organs in the peritoneal sack are placed aside. Then, an X-ray is used to confirm the disc replacement level and a marker is placed at the site. The disc is exposed after moving the large blood vessels carefully present in front of the spine.

The intervertebral lumbar disc is excised to allow access to the back of the disc and to give a clear way to ensure proper movement of the artificial disc. X-ray is then used to determine the lordosis and the appropriate size. This should be done very carefully to ensure the accurate placement of the artificial disc.

Space created is then replaced with the metallic end plates by setting the position with the help of protruding teeth and the inherent pressure of the spine. While performing the procedure, the placement is checked conscientiously, millimeter to millimeter, for long-term functioning of the replaced disc.

The central core is inserted carefully after placing the two metallic endplates at their sites. The large blood vessels and organs are moved back to their place. Lastly, dissoluble sutures are used to stitch the incisions.
