Infertility affects as much as 15% of couples around the world. These statistics explain the rising demand for IVF and other ART (Assisted reproduction techniques) treatments across all countries. Moreover, IVF is the only chance at parenthood for same-sex couples, which further contributes to the popularity of IVF procedures

Unfortunately, these treatments do not come at budget-friendly costs, especially in countries like the USA, UK, or Europe. The restrictions and stringent laws concerning IVF, egg/sperm donation, and surrogacy in some countries also make it difficult for reproductively-challenged or same-sex couples to have a baby.

Canada serves as a perfect destination for such couples. The IVF cost in Canada is quite an affordable option, and the laws offer flexibility and inclusiveness for all patients irrespective of their background or sexual orientation.

What is IVF?

  • IVF, commonly abbreviated for In-vitro fertilization, is the most common assisted reproductive technique performed on couples dealing with infertility or couples with same-sex partners.
  • The technique involves the collection of the egg from the mother’s or donor’s body and fertilizing it with the male partner’s sperm under suitable lab conditions. The sperm can also come from a donor.
  • The fertilized egg that transforms into an embryo, is then implanted back into the mother’s uterus, where it attaches itself to the uterine lining, followed by the initiation of pregnancy. However, the chances of successful pregnancy are different for each patient, determined by factors like age, health, etc.

IVF Treatment in Canada

IVF treatment in Canada begins with infertility tests for male and female partners, depending on their age and medical history.

1. Infertility Tests for Men

  • The infertility tests for men are less complicated and take up a shorter time when compared to female infertility tests. This is so because the causes of infertility in men are less than those in women.
  • Before beginning with the tests, the doctor will conduct a physical examination of the patient and dig deep into their medical history. This will include examining the patient’s genitals for varicoceles and questions concerning their lifestyle.
  • The questions include inquiring about the history of any injury, surgery, or chronic health issues, as well as the patient’s sexual habits that may affect their fertility.

Sperm and Semen analysis

This test helps the doctor analyze the quality and quantity of sperm in the patient’s sample, including the sperm’s shape, motility, and viability.

If the test results point towards the male partner being infertile, the doctor may suggest a few other tests confirming the analysis. These include:

  • Scrotal ultrasound
  • Transrectal ultrasound
  • Hormone testing
  • Post-ejaculation urinalysis
  • Genetic tests
  • Testicular biopsy
  • Specialized sperm function tests

However, if the test results suggest that the male partner has no issues concerning fertility, the doctor will move on to the diagnostic tests for the female partner.

2. Infertility Tests for Women

The initial test for a woman also begins with a physical examination and questions about their overall health, lifestyle, and medical history. Additionally, a doctor will advise one or more of the below tests:

  • Ovarian reserve testing

This test is done to determine the quality and quantity of eggs by determining the concentration of various female hormones in the blood, like follicle-stimulating hormone (FSH), estradiol (estrogen), and anti-mullerian hormone. This test is conducted during the initial days of the patient’s menstrual cycle.

  • Infectious disease screening

This test is done to screen the patient for infectious diseases like HIV. This test is also advised to the male partner.

  • Mock embryo transfer

This test helps to measure the depth of the uterine cavity and decide on the best possible route and location to implant the embryo after fertilization.

  • Uterine exam

A uterine exam aims to examine the lining of the patient’s uterus. It usually involves a:

  1. Sonohysterography: The doctor injects a fluid into the patient’s uterus through the cervix to create ultrasound images of the uterine cavity.
  2. Hysteroscopy: The doctor inserts a thin, flexible, lighted telescope through the cervix to examine the uterine cavity.

3. Ovarian stimulation

  • The doctor will suggest a patient either go for a long protocol or an antagonist protocol for ovarian stimulation, depending on their condition and medical history. Both these protocols are designed to administer drugs to stimulate the ovary for egg production.
  • The long protocol requires the patient to start receiving the injections from the 21st day of their menstrual cycle. They would also be called back to receive another set of injections during the second or third day of the next menstrual cycle.
  • The antagonist protocol follows the administration of injections starting from the 2nd day of the menstrual cycle.
  • The patient would be expected to take these injections regularly at the exact same time. If they can’t travel to the clinic daily, a nurse may also guide the patient on how to inject the medicines themselves.

4. Ultrasound examination

An ultrasound examination helps a doctor to evaluate the response of medicines on the ovary as well as egg production. If the ovaries aren’t responding well to the medicines, the doctor will advise the patient to stop the treatment.

5. HCG/GnRH Injection

HCG or GnRH are hormone injections that aid in the maturation of eggs to prepare them for retrieval after a period of 34 to 36 hours later.

6. Egg Collection

  • A doctor will proceed with egg retrieval using transvaginal sonography under general anesthesia. The procedure usually lasts for 30 minutes.
  • The number of eggs collected from the ovary depends on their response to the injections and hormones.
  • The patient can leave the clinic after 2 hours of the successful completion of the procedure. They may also experience vaginal spotting and abdominal discomfort, although these symptoms settle after 3-4 days.

7. Sperm collection

  • The male partner would be requested to produce a semen sample on the scheduled day of collection. Due to personal reasons and comfort, the patients can also bring a semen sample collected elsewhere within 30 minutes of collection.
  • Patients can also opt for a sperm freezing facility if they won’t be physically present on the day of collection.
  • Male partners should follow 3-5 days of abstinence to produce the best semen sample for collection.

8. Fertilization and embryo transfer

  • Following sperm and egg collection, the eggs will be fertilized using IVF or ICSI method under suitable lab conditions. The fertilized eggs soon transform into a zygote, and finally, an embryo.
  • The well-developed embryos will be selected for embryo transfer. This is performed 2, 3, or 5 days after egg collection.
  • The doctor will transfer the embryos into the uterine cavity with the help of sonography. Usually, 3 well-developed embryos are selected for the transfer, but this number can vary for each patient.
  • After the transfer, the patient will be prescribed a course of medication to support the luteal phase.

9. Pregnancy test

Approximately 14 days after the embryo transfer, the patient will undergo a beta-hCG test to confirm the pregnancy. A positive test will be followed by a vaginal ultrasound scan to analyze the gestation sac. If the test comes out to be negative, the patient will either use the stored embryos from the previous cycle or start the procedure again.

Precautions after an IVF treatment

  • Avoid any form of strenuous exercise, heavy lifting, and jarring movements. This will include cycling, jogging, or other vigorous sports.
  • Show compliance with the instructions shared by the doctor or the IVF specialist.
  • Take all necessary prescribed medication that can help a patient to achieve positive pregnancy results. A patient should not take any medication that’s not prescribed by the doctor. If needed, it should be discussed with the doctor well in advance.
  • Avoid smoking, drinking, and using recreational drugs.

The success rate of IVF in Canada

Patients who fall within the age group of 36 to 41 years have 25%, and those above 42 years of age have a 4% chance of success with the procedure. The success rate of an IVF procedure depends on many factors, including:
