Eligibility criteria for Zambians applying for Indian Medical Visa

Conditions that must be met by an Zambian passport holder in order to get Medical visa are:
  • Medical visa is issued exclusively for the purpose of travelling to India for specified medical treatment.
  • The list of treatments that are of primary consideration for medical visa acceptance include –
Cardiac surgery Neurosurgery Treatment of kidney disorders
Organ transplantation Joint replacement procedures Treatment of Congenital disorders
Cancer Therapy
This is a non-exhaustive list. 
  • The patient should be seeking treatment from a government recognized and reputed hospital/healthcare/medical facility that is located in India.
  • Maximum two companions/attendants are allowed to accompany the patient, who travel on Medical Attendant Visa. The attendants should be a blood relatives or close associates of the patient.
  • Yellow fever vaccination Certificate is a mandatory document for people travelling from specific countries.

List of documents required to be submitted for Indian Medical Visa Application

  • A signed, hard copy of the Indian Visa Application form of the applicant completely filled by the applicant.
  • Original passport with min. 6 months of validity and two blank pages for Indian visa stamp.
  • Applicant’s two recent photographs of passport size
  • Photocopy of the passport 
  • Yellow fever vaccination certificate must be carried by Zambians travelling to India along with a photocopy. The certificate should be taken minimum 14 days before the arrival date.
  • NRC/Residency Permit along with its photocopy
  • Proof of permanent residence of the applicant
  • An official referral letter from the local doctor/hospital in Zambia stating the nature of illness of the patient and recommending treatment in India.
  • An official visa invitation/assistance letter from a reputed Indian hospital stating the name, contact details and passport number of the patient as well as the attendant along with the treatment details. The invitation letter should be made on the letterhead of the hospital having the official stamp and signature of the designated hospital authority.
  • Copy of the attendant’s passport and their proof of relationship with the patient.
  • Medical record of the patient, including all the relevant test reports and imaging scans like CT, MRI, etc.
  • Proof of availability of funds establishing the applicant’s ability to bear the cost of treatment and stay for the duration of procedure and recovery.

  • Minor’s Birth certificate copy
  • Passport Copy of both the parents
  • Visa application has to be signed by both parents and notarized

Steps for filling Online Visa Application Form

  • Go to Indian visa official website https://indianvisaonline.gov.in/visa/index.html and click on ‘Regular Visa Application’ to start your application.
  • Fill in the required details in relevant field displayed on the page. Select the correct country name and fill in your nationality, Indian Mission/High Commission, your date of birth and email ID along with the Visa type (Medical/Medical Attendant visa), purpose of visit (Medical Treatment of Self / Medical Attendant to Patient) and other details on this page. Enter the verification code and click ‘Continue’.
  • There are three pages on the online medical visa application where you have to fill in your information. After completing each page, you can click on ‘Save and Continue’ to continue with the application or ‘Save and exit’ to temporarily leave the application. Note your Temporary Application ID before choosing Save and Exit as you will require it for accessing and completing your ‘Partially Filled application’. 
  • On the first page, fill in your personal information like your surname, first name, National ID/ Citizenship No., educational qualification etc. and then proceed to fill the details of your valid passport and your address details (present and permanent). In the following section provide family details and your occupation details. Click on ‘Save and Continue’
  • On the next page, provide information regarding the type of visa you are applying for – i.e. Medical/Medical Attendant visa and details of your medical condition, the referring hospital in your country and the concerned hospital in India where you are seeking treatment. Then fill up details for the duration of visa, the purpose of your visit, expected date of arrival and departure, previous visits to India and other countries.
  • Then in the next section, provide information of your reference in India – name, address and phone number, and details of reference in your home country.
  • In case you do not have a contact for reference in India and plan to travel to through Lyfboat, we can assist you. Contact us or use Lyfboat’s address and phone number as your reference in India while filling the application.

  • Then on the new window upload your recent passport size photo by clicking on the “Upload Photo” and choosing the appropriate file. Or you can paste it on the printed copy of the application form.
  • After completely filling the third page, click on ‘Save and Continue’ and a new window with a preview of your application will appear. On this page, check the details that you have filled in. If you need to correct or change any detail, click on Modify/Edit to edit or click on ‘Verified and Continue’ if all the information on the screen appears to be accurate.
  • The application is then submitted and registration is complete. You can now select an appointment date that is suitable to you for submission of the hard copy of your visa application at the designated Indian Mission.
  • After selecting the date click on ‘Confirm Appointment’. Another window will open up having two options – Print or Save. Click on the Save option to preserve your filled application form and not on ‘Print” as you might lose the application by doing so. Go to the home page and select ‘Print Registered Application’ there to take out the print of your visa application.
  • Put your signature on the printed application – one in the box below the photograph and another in the box at the bottom of the second page.
  • Finally, submit a hardcopy of the completed online visa application form along with the mandatory documents at the Mission counter on the appointed date (mentioned on the right margin of your application) between 0900 hrs to 1100 hrs.

Nationals of Zambia can apply for eVisa. Visa is issued with a validity of 6 months and triple entries are allowed in the validity period.

Steps for filling e-Visa application

  • Visit https://indianvisaonline.gov.in/evisa/ and start filling the application
  • Put in the information regarding the name of the country you are a citizen of, your nationality, the concerned Indian Mission and the type of visa you want to apply for. Put in the data for expected date of your arrival in India, your Date of birth, email, etc. and then enter the verification code before clicking on ‘Continue’.
  • On the first page after this, fill in your personal details in the relevant section, then passport details like Passport number, date and place of issue, date of expiry,etc. 
  • You can choose ‘Save and Continue’ to continue with the application or  select ‘Save and Temporarily exit’ if you want to fill the form later.
  • On the next page, provide details of your current & permanent address, then fill in the information for Family Details sections. 
  • Then fill the details for the Visa – Type of Visa, Visa service, the places to visit, duration of visa, no. of entries and expected port of exit from India.
  • In the other section, put in the details of your previous visa or currently valid visa – whether you have visited India previously and if yes, then – enter the address of the place where you have stayed, cities that you visited, number of your last Indian visa or currently valid Indian visa no., whether the permission to visit or visa extension has been refused earlier or not – state the reason if yes.
  • Then prove information like the countries you have visited in the last 10 years and whether you have visited SAARC countries in the past 3 years. 
  • In the next section, fill in the details of your reference contact in India as well as details of reference in your country. If you are travelling through Lyfboat services and have no reference in India, you can put in Lyfboat’s office address and phone number in this section. If the complete address doesn’t fit in the space provided, use the shorter version –

  • A new page will appear after clicking on ‘Save and Continue’, consisting of a list of additional questions. Carefully go through these questions and answer by clicking on Yes or No. After this, click on ‘Save and Continue’ to proceed.
  • Next, upload your photograph by clicking on ‘Upload Photo’. Make sure that your digital photo is under the specified size limit of the requirement, i.e. jpeg format with 10 Kb to 300 Kb of size. You can resize your photograph by clicking  here or email your photograph to care@lyfboat.com if you are using Lyfboat services. 
  • Then upload the medical documents on the next page and click on ‘Confirm’.
  • A new window with a preview of your application form will be displayed. Check the information on the preview and then click on ‘Verified and Continue’ if it is correct or choose ‘Modify’ to edit the application.
  • After clicking on Verify and Continue, you will be directed to Onlive Visa Fee Payment. You can pay the visa fee by clicking on ‘Pay now’ after selecting ‘Yes’ for the Undertaking. In case you do not want to pay right then, select ‘Pay Later’. You can make payment whenever you are ready, by going to https://indianvisaonline.gov.in/evisa/ and clicking on ‘Verify Payment/ Pay e-Visa fee. Put in your Application ID and Date of Birth to access the payment portal. 

Note: Visa application is not submitted till the fee payment is made and it should be done 4 days before your journey.

  • You will receive an email from the Officials regarding your application (acceptance/rejection). You can print your visa application form by selecting the option on Indian eVisa website if you want a hardcopy.

Processing Fee for Medical Visa from Zambia to India

The fee for Indian Medical Visa from Zambia that is valid for 6 months (Single/multiple entry) is USD 63.10 (ZMW 810) and for visa validity of upto 1 year (single/multiple entries) the fee is USD 93.48 (ZMW 1200). Medical attendant visa fee is also the same.

Processing Time for Medical Visa from Zambia to India

Indian Medical Visa from Zambia is usually issued within 3 working days after the day of submission of application. 

To track your visa status, go to the homepage of the official website https://indianvisaonline.gov.in/visa/index.html and click on ‘Visa Status Enquiry’

Fill in the Application ID provided on the receipt issued by Indian Mission/ Agent after submission of your application at the Mission counter.

After arriving in India

FRRO Registration

Foreign nationals visiting India are required to register themselves at the regulatory body – Foreign Regional Registration Office (FRRO)/ Foreign Registration Office (FRO) within 14 days of their arrival into the country. 

This is applicable to all foreigners that are above 16 years of age. You can register by reporting for registration yourself or through an authorized representative at the Foreign Registration Office.

Online registration can also be done – visit the official government website https://indianfrro.gov.in/frro/ and fill in the registration form and make payment. You have to upload the required documents and your recent photograph while applying online. 

Fine for late registration is US$ 30/ (This fee might get revised over time)

Documents that are required for registration:

  • Original and photocopy of the passport 
  • Original and photocopy of the visa
  • Applicant’s four recent passport size photographs 
  • Residence Proof
  • Official letter from the concerned Indian hospital where the patient is seeking treatment along with the relevant medical documents and duration of expected stay. If the applicant is already admitted at the hospital, then a medical certificate consisting of applicant’s attested photo and a certificate from the doctor must be submitted.

Indian Medical Visa validity and Visa extension

The Indian Medical Visa is issued by the government for a period of six months, one year or for the duration of treatment – whichever is more. Visa holder may be allowed single, tripe or multiple entry on the visa during the validity period. The visa validity is observed from the date of issue of the visa and not from the date of arrival into the country.

You can apply for the extension of visa validity in case the treatment and recovery is taking more than expected time. For this, submit the visa extension application along with the required documents to the State Government/Foreign Regional Registration Office (FRRO). The extension is finally granted by the Ministry of Home Affairs (MHA).

  • Original and photocopy of the passport 
  • Official letter and medical certificate from the medical authority stating the treatment details and duration of stay in the country for recovery.
  • 4 passport size photographs of the applicant 
  • Proof of residence in India 
  • If the applicant is still admitted in the hospital, then a medical certificate with attested photo of the applicant along with the certification by the concerned doctor has to be submitted.
