Candidates for nose job surgery

In general, ideal candidates for nose reshaping should be in good overall health and should not smoke because smoking can lead to poor blow flow and increase the risk of tissue damage. There is no specific age limit for nose surgery.

However, doctors prefer candidates for rhinoplasty who have complete facial growth. In teens and young adults, facial growth is an important consideration as it can affect the look and function of the nose over time. However, pediatric patients who have a facial birth defect, such as cleft palate or physical abnormality that may affect its function, can also undergo rhinoplasty.

Rhinoplasty procedure technique 

Open Rhinoplasty

Open rhinoplasty surgery is a surgical method in which an open incision is made on the nose to provide the plastic surgeon access to the nasal structures. The surgeon makes the incision underneath the nose in an inconspicuous location, usually between the nostrils. The cartilage and nasal bone is then exposed for the nose surgery.

The flaps of nasal skin is lifted up and off the nose, enabling the surgeon to work freely. Before starting an open nose surgery, local anesthesia is injected into the nose – around the incision site, on the nasal septum, the thin wall of nasal bone and cartilage that divides the nostrils.

The surgery is then performed on the basis of the work needed on the nose and individual’s specific needs. The surgeon may reshape the tip of the nose, straighten a deviated nasal bridge or nasal septum, or remove the hump on the nose as well as other nasal repairs.

Septoplasty is usually preferred for fixing a deviated septum. This problem occurs when the midline of the nasal septum deviates or turns to one side and looks off-center. This can cause obstructed breathing. 

Closed Rhinoplasty

The closed rhinoplasty method provides the advantage that it doesn’t leave visible scars after the surgery as the incisions are made inside the nose as the nasal bone, cartilage, or septum that is being operated on. Also, after the closed rhinoplasty it is expected to have less swelling than the open procedure.

In a closed rhinoplasty surgery, an incision is made in the nostril lining to allow the surgeon to access the bone structure underneath. It is a less invasive procedure than an open rhinoplasty surgery. However, the procedure has its limitations. Although, It requires the nasal skin to be stretched greatly, it limits the extent to which the nasal tip and skin can be reshaped. This type of rhinoplasty surgery is not recommended for patients who wish to have a revision rhinoplasty.

The closed rhinoplasty procedure has many benefits including less post-surgery swelling and no visible scars, it usually does not give as excellent results as the open method of rhinoplasty surgery. This is mainly because open rhinoplasty allows the surgeon full exposure and visuals of the nose structure.

Endoscopic Rhinoplasty

This type of surgical rhinoplasty involves the use of an endoscope, a tube that has a camera and light attached to it. This equipment enables the surgeon to see inside part of the nose while performing the surgery. Endoscopic rhinoplasty procedure is a less invasive procedure. 

It can be used in combination with the closed rhinoplasty surgery. Endoscopic rhinoplasty usually results in less swelling compared to an open rhinoplasty procedure.

Rhinoplasty Recovery

Rhinoplasty is usually performed as an outpatient procedure. Most patients can leave the hospital some day, depending on the anesthesia used and the patient’s health. After the procedure, patients is shifted to a recovery room where their recovery is monitored. Some patients, generally children, may have to stay at the hospital for 1 or 2 nights. 

After an open rhinoplasty, a splint is usually put on the outside of the nose. This remains for about 7 days post-surgery to protect and support the altered nasal framework as it heals. The bruising and swelling around the eyes dissipate gradually over the period of a month following the procedure. The numbness around the nose and month can also be expected, but it subsides within two weeks after the surgery.

Once the cast is removed, the patient start using makeup. However, complete healing of the nose will take up to a year or 18 months. In general, the recovery period for an open rhinoplasty surgery is two to four weeks. It is important to follow all the post-operative instructions and precautions during this period. Strenuous activities should be avoided for a few weeks after this cosmetic surgery.

Patients who have closed rhinoplasty surgery may experience less swelling, and bruising. They are also expected to have a much shorter recovery time. They can return to their normal activities in about two or three days after surgery. Usually, patients resume work in approximately 5 to 7 days, depending on the nature of the job.

Post-operative care after rhinoplasty:

Patients are given instructions for the postoperative care during the discharge. Some of these are:

  • After a rhinoplasty surgery (of any type), patients may experience nasal drainage and will therefore need to change their dressing until it ceases
  • It is recommended to should refrain from blowing the nose for 14 days following the operation
  • Use ice packs to alleviate swelling and bruising. It can used for the cheeks and eyes during the first two days post surgery
  • Use saline nasal spray to moisten the nose or apply vaseline with a Q-tip
  • Keep the head elevated for 48 hours after the surgery to reduce swelling
  • Avoid move the face too much such as excessive chewing or jerking the head
  • Gently brush the teeth using soft toothbrush
  • Avoid moving the upper lip as much as possible so that the nose can rest and heal
  • Avoid strenuous activities such as exercise and sex to prevent bleeding
