What is Rhinoplasty?

Rhinoplasty is a cosmetic procedure that aims to enhance the facial aesthetic by making the desirable changes to the structure and proportions of the nose. The procedure might also be used for medical reason, which is to treat structural defects in the nose that obstructs the airway.

The nose surgery can be used for various purposes, including

  • To correct the nose size to optimize of facial balance
  • To alter the nose width at the bridge or change the size and position of the nostrils
  • To correct the profile of the nose to remove visible humps or depressions on the bridge
  • Surgery for nasal tip that is enlarged, upturned, drooping, or hooked
  • Surgery for align nostrils that are large, wide or upturned
  • To correct the nasal symmetry

In case a person wants more symmetrical nose, it is best to keep in mind that facial asymmetric is present, to some degree, in everyone. Therefore, the outcome of the surgery might not be a complete symmetry, but the goal is to create as much facial balance and have a proportionate feature.

Rhinoplasty for deviated septum correction: Nose surgery can be done to improve the impaired breathing due to structural nasal defects. After careful evaluation of the nasal structure, and the problem it is causing to airflow and breathing, a person might be recommended this procedure. The correction of a deviated septum is achieved by making adjustments and alterations to the nasal structure to create a better alignment.

The benefits and limitations of rhinoplasty surgery:

Rhinoplasty is now considered as one of the most effective and commonly performed cosmetic surgeries. However, it is a complex procedure and must be performed by a trained specialist.

As with every surgery, there are a few risks associated with rhinoplasty too. The doctor carefully evaluate the nasal structure and understands the expectation before suggesting the suitable cosmetic procedure.

Some benefits of rhinoplasty include:

  • It can change the aesthetic appearance and the overall appearance of the face.
  • Can be used to correct congenital defects, structural deformities or injuries to the nose that obstructs the breathing
  • The healing process is mostly uncomplicated
  • Helps people who wish to enhance their features. This is considered after a psychological evaluation and determining that the patient doesn’t have a negative outlook towards their appearance.

Limitations of nose plastic surgery:

  • It cannot alter the whole look or personality of a person.
  • Twisted noses are not very easy to fix.

Who may consider having a rhinoplasty surgery?

An ideal candidate for rhinoplasty is a person who:

  • Has completed Facial growth is complete
  • Has a good physically health
  • Doesn’t regularly smoke
  • Has a positive outlook and realistic goals for the changes of the appearance through this surgery
  • Has structural defects that causes breathing problems

What to expect during a rhinoplasty procedure?

Pre-operative phase

The medical team at the hospital or facility will conduct some tests and medical examination to determine the suitability of the candidate for this procedure.

Review of medical history: The doctor will ask about the reason for wanting this surgery and the expected goals, if it is an elective surgery. For medical conditions, a history of nasal obstruction, previous surgeries and prescribed medications are also considered. If a person has a bleeding disorder, such as haemophilia, rhinoplasty can not be performed.

Physical examination: The doctor will conduct a physical examination to examine the facial features and the inside as well as outside part of the nose. The physical exam will enable the doctor to determine the surgical technique and plan the changes that needs to be made.

Also, the physical features, including the thickness of the skin or the strength of the cartilage (at the end of nose) may also be considered. The physical examination helps determine the impact of rhinoplasty on breathing and understand the expected outcomes.

A discussion: During the consultation, the surgeon will discuss the expectations and risks involves in the surgery. Understanding the expected goals is important for the doctor. They also explain the potential and limitation of rhinoplasty and what results should be expected.

The instructions that the candidate needs to follow before the surgery include:

  • Avoid blood thinning medications such as aspirin for two weeks before and after the surgery. These drugs may increase the risk of bleeding. The doctor may other alternative medications, if required. It is also recommended avoiding herbal remedies and over-the-counter supplements.
  • Stop or quit smoking. This is an important point as smoking can slow the healing process after the surgery. It may also make the patient more likely to have an infection.
  • Before scheduling rhinoplasty, arrange a person to drive to the facility and from the hospital to home. For the initial few days after the surgery, some people may have memory lapses, delayed reaction time and impairment of judgment. It is best if a family member or friend can stay with the patient for a night or two to help with personal care during the recovery.
  • The surgeon may also consider any additional surgery, if required, that can be done at the same time. For instance, a chin augmentation might also be preferred along with rhinoplasty. This procedure is an operation to better define the chin.

During the surgery

The basic steps involved in a rhinoplasty surgery are the following:

  • Patients are given anaesthesia, a medication to ensure comfort during the surgical procedure. The choices of anaesthesia include intravenous sedation or general anaesthesia. A suitable choice is made on the basis of doctor’s recommendation and patient’s personal preference.
  • Then the surgeon will make an incision, depending on the type of rhinoplasty operation – open or closed procedure. In a closed rhinoplasty, the incisions are made in a way that they remain hidden inside the nose.
  • Whereas in an open procedure, an incision is made across the narrow strip of tissue which separates the two nostrils, known as columella.
  • After making these incisions, the surgeon gently raises the skin that covers the nasal bones and cartilages. This gives an access to reshape the structure of the nose, as per the requirement. An enlarged or bulbous nose is reduced in size, by removing bone or cartilage from some parts.
  • The surgeon may also place additional cartilage grafts in the nose, if required. Usually, the cartilage from the nasal septum (the partition that separates the nose from the middle) is used for the grafts. Sometimes, a section of cartilage from the ear or of rib cartilage, rarely, can also be used for this purpose.
  • For correcting a deviated septum, the surgeon straightens, adjusts the alignment and reduce the projections inside the nose to improve breathing.
  • The incision is then closes, after the underlying structure of the nose has been sculpted as per the desired shape. The nasal skin and tissue are redraped and incisions are closed with sutures. There might be a need to place additional incisions in the natural creases of the nostrils to alter its size.
  • It may take some time to see the results. For a few days after the surgery, the splints and gauze packing are put to support the nose as it heals.

How is the post-operative care?

During the recovery following rhinoplasty, a splint with or without the packing might be placed inside the nose. A splint or bandages might also be placed on the outside of the nose to support and protect the newly altered structures during the initial period of healing.

Although the initial swelling may subside within a few weeks, the new nasal contour may take up to a year to fully refine. During this period, there will be gradual changes in the appearance of the nose as it refines over time and result in a more permanent outcome.

Swelling may remain and disappear or worsen in the morning during the first year after the rhinoplasty. The splint is generally removed after 5-6 days post surgery. The doctor may prescribe some medication to help with the redness, swelling, and pain, if needed.

To reduce the chances of bleeding and swelling, the patient is instructed to follow certain precautions for a few weeks after the surgery. This includes:

  • Avoid strenuous activities, including aerobics and jogging, and contact sports.
  • Avoid too much exposure to the sun, and over consumption of salt.
  • Take baths instead of showers in the initial days, as there will be bandages on the nose.
  • Do not blow the nose.
  • Add more high-fiber foods, such as fruits and vegetables, into the diet to avoid constipation. This is because constipation may cause strain which will result in putting pressure on the surgery site.
  • Avoid excessive facial expressions, including smiling or laughing.
  • Gently brush the teeth to limit the movement of upper lip.
  • Wear clothes that can be fastened in the front and doesn’t require pulling over the head, such as shirts or sweaters.
  • It is also recommended to not rest the eyeglasses or sunglasses on the nose for at least four weeks following the surgery. This is to avoid pressure on the nose. A person tape the glasses to the forehead until the nose has healed enough.
